General Data Protection Regulation Policy (updated 20/07/20)
With the new changes in the law regarding data protection (https://ico.org.uk/for-organisations/guide-to-the-general-data-protection-regulation-gdpr/) I need to request your permission to collect, record and use you personal information. GDPR is a European-wide update of the data protection laws last changed in the 1990’s.
The information that I collect on the consultation form is to allow me to treat you in the safest way and to decide which treatment is most suitable to you. I also ask for a way of communicating e.g. mobile number or email so that I can update you of any changes to appointments and from time to time with a newsletter. I request address, name and date of birth as a way of identifying you to keep your record of treatment correct and up-to-date.
If you have contacted me then those conversations are kept either in that form e.g. text message/ Whats app/ Facebook/ email / forms completed etc. This allows me to check back on any arrangements we have made, records types of treatments requested and allows me to get back in contact with you within the parameters of those conversations.
This information is kept on paper. It is stored in a locked box and kept in my treatment room. I have a paper calendar and a calendar on my mobile where I record your name, treatment, time and price. I may contact you by phone or email if you have consented to this or contacted me in this way then your name and number or email address will be stored on phone/ email. My phone and computer are password protected so only I can gain access to these details.
I store the records for as long as necessary. I do not share or sell your details to anybody, they are there for my reference only. If I feel it is in your interest to be referred to another practitioner I will request separate permission from you to pass your information to that person.
If you no longer want to be contacted by me you can speak to me, email, call or text and I will remove your contact details from my file. Your consultation form will be kept in the locked box as this is required for my insurance purposes.
You are entitled to request a copy of your records or to look at the record that I hold on you. Please allow 30 days for me to process this request, although my aim is to provide these to you as promptly as possible.
If you need to make a complaint please contact me in the first instance by speaking to me directly, if this is not possible or you feel your problem was not resolved satisfactorily please put your concerns into writing and either post to me or email. I will then do my best to come to a satisfactory resolution with you. If this has not been possible the next step will be to refer to my insurers and my legal team.
My website uses Google Analytics, this means that you cannot be identified as an individual from the data collected.
I am registered with the ICO which the Data Protection Act requires all businesses to do.
